Boeing convinced itself that their product was extremely safe.
I've made my own effort to imagine how this process went so far "Off the rails" and what might have prevented the catastrophe.
In the development process, I suspect that most of the actors believed that the system was safe; there seems to have been a sort of shared illusion that all was OK. To most of the people involved in the development process, in their "Corner of the elephant" everything seemed to be in order.
Independence visibility across the range of departments and teams involved in the process.
Boeing needed somebody monitoring MCAS, empowered to stand up and shout "Stop everything!".
